afs: Don't use folio->private to record partial modification

AFS currently uses folio->private to store the range of bytes within a
folio that have been modified - the idea being that if we have, say, a 2MiB
folio and someone writes a single byte, we only have to write back that
single page and not the whole 2MiB folio - thereby saving on network
bandwidth.

Remove this, at least for now, and accept the extra network load (which
doesn't matter in the common case of writing a whole file at a time from
beginning to end).

This makes folio->private available for netfslib to use.

netfs: Move pinning-for-writeback from fscache to netfs

Move the resource pinning-for-writeback from fscache code to netfslib code.
This is used to keep a cache backing object pinned whilst we have dirty
pages on the netfs inode in the pagecache such that VM writeback will be
able to reach it.

Whilst we're at it, switch the parameters of netfs_unpin_writeback() to
match ->write_inode() so that it can be used for that directly.

Note that this mechanism could be more generically useful than that for
network filesystems. Quite often they have to keep around other resources
(e.g. authentication tokens or network connections) until the writeback is
complete.

afs: Fix accidental truncation when storing data

When an AFS FS.StoreData RPC call is made, amongst other things it is
given the resultant file size to be. On the server, this is processed
by truncating the file to new size and then writing the data.

Now, kafs has a lock (vnode->io_lock) that serves to serialise
operations against a specific vnode (ie. inode), but the parameters for
the op are set before the lock is taken. This allows two writebacks
(say sync and kswapd) to race - and if writes are ongoing the writeback
for a later write could occur before the writeback for an earlier one if
the latter gets interrupted.

Note that afs_writepages() cannot take i_mutex and only takes a shared
lock on vnode->validate_lock.

Also note that the server does the truncation and the write inside a
lock, so there's no problem at that end.

Fix this by moving the calculation for the proposed new i_size inside
the vnode->io_lock. Also reset the iterator (which we might have read
from) and update the mtime setting there.

mm: return an ERR_PTR from __filemap_get_folio

Instead of returning NULL for all errors, distinguish between:

- no entry found and not asked to allocated (-ENOENT)
- failed to allocate memory (-ENOMEM)
- would block (-EAGAIN)

so that callers don't have to guess the error based on the passed in
flags.

Also pass through the error through the direct callers: filemap_get_folio,
filemap_lock_folio filemap_grab_folio and filemap_get_incore_folio.

afs: Stop implementing ->writepage()

We're trying to get rid of the ->writepage() hook[1]. Stop afs from using
it by unlocking the page and calling afs_writepages_region() rather than
folio_write_one().

A flag is passed to afs_writepages_region() to indicate that it should only
write a single region so that we don't flush the entire file in
->write_begin(), but do add other dirty data to the region being written to
try and reduce the number of RPC ops.

This requires ->migrate_folio() to be implemented, so point that at
filemap_migrate_folio() for files and also for symlinks and directories.

This can be tested by turning on the afs_folio_dirty tracepoint and then
doing something like:

xfs_io -c "w 2223 7000" -c "w 15000 22222" -c "w 23 7" /afs/my/test/foo

and then looking in the trace to see if the write at position 15000 gets
stored before page 0 gets dirtied for the write at position 23.

use less confusing names for iov_iter direction initializers

READ/WRITE proved to be actively confusing - the meanings are
"data destination, as used with read(2)" and "data source, as
used with write(2)", but people keep interpreting those as
"we read data from it" and "we write data to it", i.e. exactly
the wrong way.

Call them ITER_DEST and ITER_SOURCE - at least that is harder
to misinterpret...

afs: Enable multipage folio support

Enable multipage folio support for the afs filesystem.

Support has already been implemented in netfslib, fscache and cachefiles
and in most of afs, but I've waited for Matthew Wilcox's latest folio
changes.

Note that it does require a change to afs_write_begin() to return the
correct subpage. This is a "temporary" change as we're working on
getting rid of the need for ->write_begin() and ->write_end()
completely, at least as far as network filesystems are concerned - but
it doesn't prevent afs from making use of the capability.

netfs: Further cleanups after struct netfs_inode wrapper introduced

Change the signature of netfs helper functions to take a struct netfs_inode
pointer rather than a struct inode pointer where appropriate, thereby
relieving the need for the network filesystem to convert its internal inode
format down to the VFS inode only for netfslib to bounce it back up. For
type safety, it's better not to do that (and it's less typing too).

Give netfs_write_begin() an extra argument to pass in a pointer to the
netfs_inode struct rather than deriving it internally from the file
pointer. Note that the ->write_begin() and ->write_end() ops are intended
to be replaced in the future by netfslib code that manages this without the
need to call in twice for each page.

netfs_readpage() and similar are intended to be pointed at directly by the
address_space_operations table, so must stick to the signature dictated by
the function pointers there.

netfs: Fix gcc-12 warning by embedding vfs inode in netfs_i_context

While randstruct was satisfied with using an open-coded "void *" offset
cast for the netfs_i_context <-> inode casting, __builtin_object_size() as
used by FORTIFY_SOURCE was not as easily fooled. This was causing the
following complaint[1] from gcc v12:

Fix this by embedding a struct inode into struct netfs_i_context (which
should perhaps be renamed to struct netfs_inode). The struct inode
vfs_inode fields are then removed from the 9p, afs, ceph and cifs inode
structs and vfs_inode is then simply changed to "netfs.inode" in those
filesystems.

Further, rename netfs_i_context to netfs_inode, get rid of the
netfs_inode() function that converted a netfs_i_context pointer to an
inode pointer (that can now be done with &ctx->inode) and rename the
netfs_i_context() function to netfs_inode() (which is now a wrapper
around container_of()).

Most of the changes were done with:

Kees suggested doing it with a pair structure[2] and a special
declarator to insert that into the network filesystem's inode

matter if struct randomisation reorders things.

Dave Chinner suggested using a filesystem-specific VFS_I() function in
each filesystem to convert that filesystem's own inode wrapper struct
into the VFS inode struct[4].

Version #2:
- Fix a couple of missed name changes due to a disabled cifs option.
- Rename nfs_i_context to nfs_inode
- Use "netfs" instead of "nic" as the member name in per-fs inode wrapper
structs.

[ This also undoes commit 507160f46c55 ("netfs: gcc-12: temporarily
disable '-Wattribute-warning' for now") that is no longer needed ]

afs: Adjust ACK interpretation to try and cope with NAT

If a client's address changes, say if it is NAT'd, this can disrupt an in
progress operation. For most operations, this is not much of a problem,
but StoreData can be different as some servers modify the target file as
the data comes in, so if a store request is disrupted, the file can get
corrupted on the server.

The problem is that the server doesn't recognise packets that come after
the change of address as belonging to the original client and will bounce
them, either by sending an OUT_OF_SEQUENCE ACK to the apparent new call if
the packet number falls within the initial sequence number window of a call
or by sending an EXCEEDS_WINDOW ACK if it falls outside and then aborting
it. In both cases, firstPacket will be 1 and previousPacket will be 0 in
the ACK information.

Fix this by the following means:

(1) If a client call receives an EXCEEDS_WINDOW ACK with firstPacket as 1
and previousPacket as 0, assume this indicates that the server saw the
incoming packets from a different peer and thus as a different call.
Fail the call with error -ENETRESET.

(2) Also fail the call if a similar OUT_OF_SEQUENCE ACK occurs if the
first packet has been hard-ACK'd. If it hasn't been hard-ACK'd, the
ACK packet will cause it to get retransmitted, so the call will just
be repeated.

(3) Make afs_select_fileserver() treat -ENETRESET as a straight fail of
the operation.

(4) Prioritise the error code over things like -ECONNRESET as the server
did actually respond.

(5) Make writeback treat -ENETRESET as a retryable error and make it
redirty all the pages involved in a write so that the VM will retry.

Note that there is still a circumstance that I can't easily deal with: if
the operation is fully received and processed by the server, but the reply
is lost due to address change. There's no way to know if the op happened.
We can examine the server, but a conflicting change could have been made by
a third party - and we can't tell the difference. In such a case, a
message like:

kAFS: vnode modified {100058:146266} b7->b8 YFS.StoreData64 (op=2646a)

will be logged to dmesg on the next op to touch the file and the client
will reset the inode state, including invalidating clean parts of the
pagecache.

afs: Maintain netfs_i_context::remote_i_size

Make afs use netfslib's tracking for the server's idea of what the current
inode size is independently of inode->i_size. We really want to use this
value when calculating the new vnode size when initiating a StoreData RPC
op rather than the size stat() presents to the user (ie. inode->i_size) as
the latter is affected by as-yet uncommitted writes.

The netfs_i_context struct so far contains a single field for the network
filesystem to use - the cache cookie:

afs: Fix potential thrashing in afs writeback

In afs_writepages_region(), if the dirty page we find is undergoing
writeback or write to cache, but the sync_mode is WB_SYNC_NONE, we go
round the loop trying the same page again and again with no pausing or
waiting unless and until another thread manages to clear the writeback
and fscache flags.

Fix this with three measures:

(1) Advance start to after the page we found.

(2) Break out of the loop and return if rescheduling is requested.

(3) Arbitrarily give up after a maximum of 5 skips.

afs: Copy local writes to the cache when writing to the server

When writing to the server from afs_writepage() or afs_writepages(), copy
the data to the cache object too.

To make this possible, the cookie must have its active users count
incremented when the page is dirtied and kept incremented until we manage
to clean up all the pages. This allows the writeback to take place after
the last file struct is released.

afs: Convert afs to use the new fscache API

Change the afs filesystem to support the new afs driver.

The following changes have been made:

(1) The fscache_netfs struct is no more, and there's no need to register
the filesystem as a whole. There's also no longer a cell cookie.

(2) The volume cookie is now an fscache_volume cookie, allocated with
fscache_acquire_volume(). This function takes three parameters: a
string representing the "volume" in the index, a string naming the
cache to use (or NULL) and a u64 that conveys coherency metadata for
the volume.

For afs, I've made it render the volume name string as:

"afs,<cell>,<volume_id>"

and the coherency data is currently 0.

(3) The fscache_cookie_def is no more and needed information is passed
directly to fscache_acquire_cookie(). The cache no longer calls back
into the filesystem, but rather metadata changes are indicated at
other times.

fscache_acquire_cookie() is passed the same keying and coherency
information as before, except that these are now stored in big endian
form instead of cpu endian. This makes the cache more copyable.

(4) fscache_use_cookie() and fscache_unuse_cookie() are called when a file
is opened or closed to prevent a cache file from being culled and to
keep resources to hand that are needed to do I/O.

fscache_use_cookie() is given an indication if the cache is likely to
be modified locally (e.g. the file is open for writing).

fscache_unuse_cookie() is given a coherency update if we had the file
open for writing and will update that.

(5) fscache_invalidate() is now given uptodate auxiliary data and a file
size. It can also take a flag to indicate if this was due to a DIO
write. This is wrapped into afs_fscache_invalidate() now for
convenience.

(6) fscache_resize() now gets called from the finalisation of
afs_setattr(), and afs_setattr() does use/unuse of the cookie around
the call to support this.

(7) fscache_note_page_release() is called from afs_release_page().

(8) Use a killable wait in nfs_vm_page_mkwrite() when waiting for
PG_fscache to be cleared.

Render the parts of the cookie key for an afs inode cookie as big endian.

mm/writeback: Add folio_wait_writeback()

wait_on_page_writeback_killable() only has one caller, so convert it to
call folio_wait_writeback_killable(). For the wait_on_page_writeback()
callers, add a compatibility wrapper around folio_wait_writeback().

Turning PageWriteback() into folio_test_writeback() eliminates a call
to compound_head() which saves 8 bytes and 15 bytes in the two
functions. Unfortunately, that is more than offset by adding the
wait_on_page_writeback compatibility wrapper for a net increase in text
of 7 bytes.

afs: Fix updating of i_blocks on file/dir extension

When an afs file or directory is modified locally such that the total file
size is extended, i_blocks needs to be recalculated too.

Fix this by making afs_write_end() and afs_edit_dir_add() call
afs_set_i_size() rather than setting inode->i_size directly as that also
recalculates inode->i_blocks.

This can be tested by creating and writing into directories and files and
then examining them with du. Without this change, directories show a 4
blocks (they start out at 2048 bytes) and files show 0 blocks; with this
change, they should show a number of blocks proportional to the file size
rounded up to 1024.

afs: Add missing vnode validation checks

afs_d_revalidate() should only be validating the directory entry it is
given and the directory to which that belongs; it shouldn't be validating
the inode/vnode to which that dentry points. Besides, validation need to
be done even if we don't call afs_d_revalidate() - which might be the case
if we're starting from a file descriptor.

In order for afs_d_revalidate() to be fixed, validation points must be
added in some other places. Certain directory operations, such as
afs_unlink(), already check this, but not all and not all file operations
either.

Note that the validation of a vnode not only checks to see if the
attributes we have are correct, but also gets a promise from the server to
notify us if that file gets changed by a third party.

Add the following checks:

- Check the vnode we're going to make a hard link to.
- Check the vnode we're going to move/rename.
- Check the vnode we're going to read from.
- Check the vnode we're going to write to.
- Check the vnode we're going to sync.
- Check the vnode we're going to make a mapped page writable for.

Some of these aren't strictly necessary as we're going to perform a server
operation that might get the attributes anyway from which we can determine
if something changed - though it might not get us a callback promise.

afs: Fix page leak

There's a loop in afs_extend_writeback() that adds extra pages to a write
we want to make to improve the efficiency of the writeback by making it
larger. This loop stops, however, if we hit a page we can't write back
from immediately, but it doesn't get rid of the page ref we speculatively
acquired.

This was caused by the removal of the cleanup loop when the code switched
from using find_get_pages_contig() to xarray scanning as the latter only
gets a single page at a time, not a batch.

Fix this by putting the page on a ref on an early break from the loop.
Unfortunately, we can't just add that page to the pagevec we're employing
as we'll go through that and add those pages to the RPC call.

This was found by the generic/074 test. It leaks ~4GiB of RAM each time it
is run - which can be observed with "top".

Fix afs_write_end() to correctly handle a short copy into the intended
write region of the page. Two things are necessary:

(1) If the page is not up to date, then we should just return 0
(ie. indicating a zero-length copy). The loop in
generic_perform_write() will go around again, possibly breaking up the
iterator into discrete chunks[1].

This is analogous to commit b9de313cf05fe08fa59efaf19756ec5283af672a
for ceph.

(2) The page should not have been set uptodate if it wasn't completely set
up by netfs_write_begin() (this will be fixed in the next patch), so
we need to set uptodate here in such a case.

Also remove the assertion that was checking that the page was set uptodate
since it's now set uptodate if it wasn't already a few lines above. The
assertion was from when uptodate was set elsewhere.

afs: Fix partial writeback of large files on fsync and close

In commit e87b03f5830e ("afs: Prepare for use of THPs"), the return
value for afs_write_back_from_locked_page was changed from a number
of pages to a length in bytes. The loop in afs_writepages_region uses
the return value to compute the index that will be used to find dirty
pages in the next iteration, but treats it as a number of pages and
wrongly multiplies it by PAGE_SIZE. This gives a very large index value,
potentially skipping any dirty data that was not covered in the first
pass, which is limited to 256M.

This causes fsync(), and indirectly close(), to only do a partial
writeback of a large file's dirty data. The rest is eventually written
back by background threads after dirty_expire_centisecs.

The generic/464 xfstest causes kAFS to emit occasional warnings of the
form:

kAFS: vnode modified {100055:8a} 30->31 YFS.StoreData64 (c=6015)

This indicates that the data version received back from the server did not
match the expected value (the DV should be incremented monotonically for
each individual modification op committed to a vnode).

What is happening is that a lookup call is doing a bulk status fetch
speculatively on a bunch of vnodes in a directory besides getting the
status of the vnode it's actually interested in. This is racing with a
StoreData operation (though it could also occur with, say, a MakeDir op).

On the client, a modification operation locks the vnode, but the bulk
status fetch only locks the parent directory, so no ordering is imposed
there (thereby avoiding an avenue to deadlock).

On the server, the StoreData op handler doesn't lock the vnode until it's
received all the request data, and downgrades the lock after committing the
data until it has finished sending change notifications to other clients -
which allows the status fetch to occur before it has finished.

This means that:

- a status fetch can access the target vnode either side of the exclusive
section of the modification

- the status fetch could start before the modification, yet finish after,
and vice-versa.

- the status fetch and the modification RPCs can complete in either order.

- the status fetch can return either the before or the after DV from the
modification.

- the status fetch might regress the locally cached DV.

Some of these are handled by the previous fix[1], but that's not sufficient
because it checks the DV it received against the DV it cached at the start
of the op, but the DV might've been updated in the meantime by a locally
generated modification op.

Fix this by the following means:

(1) Keep track of when we're performing a modification operation on a
vnode. This is done by marking vnode parameters with a 'modification'
note that causes the AFS_VNODE_MODIFYING flag to be set on the vnode
for the duration.

(2) Alter the speculation race detection to ignore speculative status
fetches if either the vnode is marked as being modified or the data
version number is not what we expected.

Note that whilst the "vnode modified" warning does get recovered from as it
causes the client to refetch the status at the next opportunity, it will
also invalidate the pagecache, so changes might get lost.

Make AFS use the new netfs_write_begin() helper to do the pre-reading
required before the write. If successful, the helper returns with the
required page filled in and locked. It may read more than just one page,
expanding the read to meet cache granularity requirements as necessary.

Note: A more advanced version of this could be made that does
generic_perform_write() for a whole cache granule. This would make it
easier to avoid doing the download/read for the data to be overwritten.

operations:

- afs_readpage() now hands off responsibility to netfs_readpage().

- afs_readahead() just hands off responsibility to netfs_readahead().

These make use of the cache if a cookie is supplied, otherwise just call
the ->issue_op() method a sufficient number of times to complete the entire
request.

Use a single ITER_XARRAY iterator to describe the portion of a file to be
transmitted to the server rather than generating a series of small
ITER_BVEC iterators on the fly. This will make it easier to implement AIO
in afs.

In theory we could maybe use one giant ITER_BVEC, but that means
potentially allocating a huge array of bio_vec structs (max 256 per page)
when in fact the pagecache already has a structure listing all the relevant
pages (radix_tree/xarray) that can be walked over.

afs: Set up the iov_iter before calling afs_extract_data()

afs_extract_data() sets up a temporary iov_iter and passes it to AF_RXRPC
each time it is called to describe the remaining buffer to be filled.

Instead:

(1) Put an iterator in the afs_call struct.

(2) Set the iterator for each marshalling stage to load data into the
appropriate places. A number of convenience functions are provided to
this end (eg. afs_extract_to_buf()).

This iterator is then passed to afs_extract_data().

(3) Use the new ITER_XARRAY iterator when reading data to load directly
into the inode's pages without needing to create a list of them.

This will allow O_DIRECT calls to be supported in future patches.

Pass a pointer to the page being accessed into the dirty region helpers so
that the size of the page can be determined in case it's a transparent huge
page.

data as these can be retrieved directly from the page struct.

When afs_write_end() is called with copied == 0, it tries to set the
dirty region, but there's no way to actually encode a 0-length region in

"0,0", for example, indicates a 1-byte region at offset 0. The maths
miscalculates this and sets it incorrectly.

Fix it to just do nothing but unlock and put the page in this case. We
don't actually need to mark the page dirty as nothing presumably
changed.

afs: Fix dirty-region encoding on ppc32 with 64K pages

The dirty region bounds stored in page->private on an afs page are 15 bits
on a 32-bit box and can, at most, represent a range of up to 32K within a
32K page with a resolution of 1 byte. This is a problem for powerpc32 with
64K pages enabled.

Further, transparent huge pages may get up to 2M, which will be a problem
for the afs filesystem on all 32-bit arches in the future.

Fix this by decreasing the resolution. For the moment, a 64K page will
have a resolution determined from PAGE_SIZE. In the future, the page will
need to be passed in to the helper functions so that the page size can be
assessed and the resolution determined dynamically.

Note that this might not be the ideal way to handle this, since it may
allow some leakage of undirtied zero bytes to the server's copy in the case
of a 3rd-party conflict. Fixing that would require a separately allocated
record and is a more complicated fix.

page->private when truncating a page. If the dirty region is entirely
removed, then the private data is cleared and the page dirty state is
cleared.

Without this, if the page is truncated and then expanded again by truncate,
zeros from the expanded, but no-longer dirty region may get written back to
the server if the page gets laundered due to a conflicting 3rd-party write.

It mustn't, however, shorten the dirty region of the page if that page is
still mmapped and has been marked dirty by afs_page_mkwrite(), so a flag is
stored in page->private to record this.

afs: Alter dirty range encoding in page->private

Currently, page->private on an afs page is used to store the range of
dirtied data within the page, where the range includes the lower bound, but
excludes the upper bound (e.g. 0-1 is a range covering a single byte).

This, however, requires a superfluous bit for the last-byte bound so that
on a 4KiB page, it can say 0-4096 to indicate the whole page, the idea
being that having both numbers the same would indicate an empty range.
This is unnecessary as the PG_private bit is clear if it's an empty range
(as is PG_dirty).

Alter the way the dirty range is encoded in page->private such that the
upper bound is reduced by 1 (e.g. 0-0 is then specified the same single
byte range mentioned above).

Applying this to both bounds frees up two bits, one of which can be used in
a future commit.

This allows the afs filesystem to be compiled on ppc32 with 64K pages;
without this, the following warnings are seen:

afs: Wrap page->private manipulations in inline functions

The afs filesystem uses page->private to store the dirty range within a
page such that in the event of a conflicting 3rd-party write to the server,
we write back just the bits that got changed locally.

However, there are a couple of problems with this:

(1) I need a bit to note if the page might be mapped so that partial
invalidation doesn't shrink the range.

(2) There aren't necessarily sufficient bits to store the entire range of
data altered (say it's a 32-bit system with 64KiB pages or transparent
huge pages are in use).

So wrap the accesses in inline functions so that future commits can change
how this works.

Also move them out of the tracing header into the in-directory header.
There's not really any need for them to be in the tracing header.

afs: Fix deadlock between writeback and truncate

The afs filesystem has a lock[*] that it uses to serialise I/O operations
going to the server (vnode->io_lock), as the server will only perform one
modification operation at a time on any given file or directory. This
prevents the the filesystem from filling up all the call slots to a server
with calls that aren't going to be executed in parallel anyway, thereby
allowing operations on other files to obtain slots.

[*] Note that is probably redundant for directories at least since
i_rwsem is used to serialise directory modifications and
lookup/reading vs modification. The server does allow parallel
non-modification ops, however.

When a file truncation op completes, we truncate the in-memory copy of the
file to match - but we do it whilst still holding the io_lock, the idea
being to prevent races with other operations.

However, if writeback starts in a worker thread simultaneously with
truncation (whilst notify_change() is called with i_rwsem locked, writeback
pays it no heed), it may manage to set PG_writeback bits on the pages that
will get truncated before afs_setattr_success() manages to call
truncate_pagecache(). Truncate will then wait for those pages - whilst
still inside io_lock:

Note that whilst afs_setattr() calls filemap_write_and_wait(), the fact
that the caller is holding i_rwsem doesn't preclude more pages being
dirtied through an mmap'd region.

Fix this by:

(1) Use the vnode validate_lock to mediate access between afs_setattr()
and afs_writepages():

(a) Exclusively lock validate_lock in afs_setattr() around the whole
RPC operation.

(b) If WB_SYNC_ALL isn't set on entry to afs_writepages(), trying to
shared-lock validate_lock and returning immediately if we couldn't
get it.

(c) If WB_SYNC_ALL is set, wait for the lock.

The validate_lock is also used to validate a file and to zap its cache
if the file was altered by a third party, so it's probably a good fit
for this.

(2) Move the truncation outside of the io_lock in setattr, using the same
hook as is used for local directory editing.

This requires the old i_size to be retained in the operation record as
we commit the revised status to the inode members inside the io_lock
still, but we still need to know if we reduced the file size.

afs: Fix interruption of operations

The afs filesystem driver allows unstarted operations to be cancelled by
signal, but most of these can easily be restarted (mkdir for example). The
primary culprits for reproducing this are those applications that use
SIGALRM to display a progress counter.

File lock-extension operation is marked uninterruptible as we have a
limited time in which to do it, and the release op is marked
uninterruptible also as if we fail to unlock a file, we'll have to wait 20
mins before anyone can lock it again.

The store operation logs a warning if it gets interruption, e.g.:

kAFS: Unexpected error from FS.StoreData -4

because it's run from the background - but it can also be run from
fdatasync()-type things. However, store options aren't marked
interruptible at the moment.

Fix this in the following ways:

(1) Mark store operations as uninterruptible. It might make sense to
relax this for certain situations, but I'm not sure how to make sure
that background store ops aren't affected by signals to foreground
processes that happen to trigger them.

(2) In afs_get_io_locks(), where we're getting the serialisation lock for
talking to the fileserver, return ERESTARTSYS rather than EINTR
because a lot of the operations (e.g. mkdir) are restartable if we
haven't yet started sending the op to the server.

afs: Fix truncation issues and mmap writeback size

Fix the following issues:

(1) Fix writeback to reduce the size of a store operation to i_size,
effectively discarding the extra data.

The problem comes when afs_page_mkwrite() records that a page is about
to be modified by mmap(). It doesn't know what bits of the page are
going to be modified, so it records the whole page as being dirty
(this is stored in page->private as start and end offsets).

Without this, the marshalling for the store to the server extends the
size of the file to the end of the page (in afs_fs_store_data() and
yfs_fs_store_data()).

(2) Fix setattr to actually truncate the pagecache, thereby clearing
the discarded part of a file.

(3) Fix setattr to check that the new size is okay and to disable
ATTR_SIZE if i_size wouldn't change.

(4) Force i_size to be updated as the result of a truncate.

(5) Don't truncate if ATTR_SIZE is not set.

(6) Call pagecache_isize_extended() if the file was enlarged.

Note that truncate_set_size() isn't used because the setting of i_size is
done inside afs_vnode_commit_status() under the vnode->cb_lock.

Found with the generic/029 and generic/393 xfstests.

afs: Concoct ctimes

The in-kernel afs filesystem ignores ctime because the AFS fileserver
protocol doesn't support ctimes. This, however, causes various xfstests to
fail.

Work around this by:

(1) Setting ctime to attr->ia_ctime in afs_setattr().

(2) Not ignoring ATTR_MTIME_SET, ATTR_TIMES_SET and ATTR_TOUCH settings.

(3) Setting the ctime from the server mtime when on the target file when
creating a hard link to it.

(4) Setting the ctime on directories from their revised mtimes when
renaming/moving a file.

Found by the generic/221 and generic/309 xfstests.

afs: Build an abstraction around an "operation" concept

Turn the afs_operation struct into the main way that most fileserver
operations are managed. Various things are added to the struct, including
the following:

(1) All the parameters and results of the relevant operations are moved
into it, removing corresponding fields from the afs_call struct.
afs_call gets a pointer to the op.

(2) The target volume is made the main focus of the operation, rather than
the target vnode(s), and a bunch of op->vnode->volume are made
op->volume instead.

(3) Two vnode records are defined (op->file[]) for the vnode(s) involved
in most operations. The vnode record (struct afs_vnode_param)
contains:

- The vnode pointer.

- The fid of the vnode to be included in the parameters or that was
returned in the reply (eg. FS.MakeDir).

- The status and callback information that may be returned in the
reply about the vnode.

- Callback break and data version tracking for detecting
simultaneous third-parth changes.

(4) Pointers to dentries to be updated with new inodes.

(5) An operations table pointer. The table includes pointers to functions
for issuing AFS and YFS-variant RPCs, handling the success and abort
of an operation and handling post-I/O-lock local editing of a
directory.

To make this work, the following function restructuring is made:

(A) The rotation loop that issues calls to fileservers that can be found
in each function that wants to issue an RPC (such as afs_mkdir()) is
extracted out into common code, in a new file called fs_operation.c.

(B) The rotation loops, such as the one in afs_mkdir(), are replaced with
a much smaller piece of code that allocates an operation, sets the
parameters and then calls out to the common code to do the actual
work.

(C) The code for handling the success and failure of an operation are
moved into operation functions (as (5) above) and these are called
from the core code at appropriate times.

(D) The pseudo inode getting stuff used by the dynamic root code is moved
over into dynroot.c.

(E) struct afs_iget_data is absorbed into the operation struct and
afs_iget() expects to be given an op pointer and a vnode record.

(F) Point (E) doesn't work for the root dir of a volume, but we know the
FID in advance (it's always vnode 1, unique 1), so a separate inode
getter, afs_root_iget(), is provided to special-case that.

(G) The inode status init/update functions now also take an op and a vnode
record.

(H) The RPC marshalling functions now, for the most part, just take an
afs_operation struct as their only argument. All the data they need
is held there. The result delivery functions write their answers
there as well.

(I) The call is attached to the operation and then the operation core does
the waiting.

And then the new operation code is, for the moment, made to just initialise
the operation, get the appropriate vnode I/O locks and do the same rotation
loop as before.

This lays the foundation for the following changes in the future:

(*) Overhauling the rotation (again).

(*) Support for asynchronous I/O, where the fileserver rotation must be
done asynchronously also.

When applying the status and callback in the response of an operation,
apply them in the same critical section so that there's no race between
checking the callback state and checking status-dependent state (such as
the data version).

Fix this by:

(1) Allocating a joint {status,callback} record (afs_status_cb) before
calling the RPC function for each vnode for which the RPC reply
contains a status or a status plus a callback. A flag is set in the
record to indicate if a callback was actually received.

(2) These records are passed into the RPC functions to be filled in. The
afs_decode_status() and yfs_decode_status() functions are removed and
the cb_lock is no longer taken.

(3) xdr_decode_AFSFetchStatus() and xdr_decode_YFSFetchStatus() no longer
update the vnode.

(4) xdr_decode_AFSCallBack() and xdr_decode_YFSCallBack() no longer update
the vnode.

(5) vnodes, expected data-version numbers and callback break counters
(cb_break) no longer need to be passed to the reply delivery
functions.

Note that, for the moment, the file locking functions still need
access to both the call and the vnode at the same time.

(6) afs_vnode_commit_status() is now given the cb_break value and the
expected data_version and the task of applying the status and the
callback to the vnode are now done here.

This is done under a single taking of vnode->cb_lock.

(7) afs_pages_written_back() is now called by afs_store_data() rather than
by the reply delivery function.

afs_pages_written_back() has been moved to before the call point and
is now given the first and last page numbers rather than a pointer to
the call.

(8) The indicator from YFS.RemoveFile2 as to whether the target file
actually got removed (status.abort_code == VNOVNODE) rather than
merely dropping a link is now checked in afs_unlink rather than in
xdr_decode_YFSFetchStatus().

Supplementary fixes:

(*) afs_cache_permit() now gets the caller_access mask from the
afs_status_cb object rather than picking it out of the vnode's status
record. afs_fetch_status() returns caller_access through its argument
list for this purpose also.

(*) afs_inode_init_from_status() now uses a write lock on cb_lock rather
than a read lock and now sets the callback inside the same critical
section.

afs: Make some RPC operations non-interruptible

Make certain RPC operations non-interruptible, including:

(*) Set attributes
(*) Store data

We don't want to get interrupted during a flush on close, flush on
unlock, writeback or an inode update, leaving us in a state where we
still need to do the writeback or update.

(*) Extend lock
(*) Release lock

We don't want to get lock extension interrupted as the file locks on
the server are time-limited. Interruption during lock release is less
of an issue since the lock is time-limited, but it's better to
complete the release to avoid a several-minute wait to recover it.

*Setting* the lock isn't a problem if it's interrupted since we can
just return to the user and tell them they were interrupted - at
which point they can elect to retry.

(*) Silly unlink

We want to remove silly unlink files if we can, rather than leaving
them for the salvager to clear up.

Note that whilst these calls are no longer interruptible, they do have
timeouts on them, so if the server stops responding the call will fail with
something like ETIME or ECONNRESET.

Without this, the following:

kAFS: Unexpected error from FS.StoreData -512

appears in dmesg when a pending store data gets interrupted and some
processes may just hang.

Additionally, make the code that checks/updates the server record ignore
failure due to interruption if the main call is uninterruptible and if the
server has an address list. The next op will check it again since the
expiration time on the old list has past.

Increase the sizes of the volume ID to 64 bits and the vnode ID (inode
number equivalent) to 96 bits to allow the support of YFS.

and i_generation as i_ino is not sufficiently capacious. It also requires
this data to be placed into the vnode cache key for fscache.

For the moment, just discard the top 32 bits of the vnode ID when returning
it though stat.

afs: Do better accretion of small writes on newly created content

Processes like ld that do lots of small writes that aren't necessarily
contiguous result in a lot of small StoreData operations to the server, the
idea being that if someone else changes the data on the server, we only
write our changes over that and not the space between. Further, we don't
want to write back empty space if we can avoid it to make it easier for the
server to do sparse files.

However, making lots of tiny RPC ops is a lot less efficient for the server
than one big one because each op requires allocation of resources and the
taking of locks, so we want to compromise a bit.

Reduce the load by the following:

(1) If a file is just created locally or has just been truncated with
O_TRUNC locally, allow subsequent writes to the file to be merged with
intervening space if that space doesn't cross an entire intervening
page.

(2) Don't flush the file on ->flush() but rather on ->release() if the
file was open for writing.

Just linking vmlinux.o, without this patch, looking in /proc/fs/afs/stats:

file-wr : n=441 nb=513581204

and after the patch:

file-wr : n=62 nb=513668555

there were 379 fewer StoreData RPC operations at the expense of an extra
87K being written.

Add statistics to /proc/fs/afs/stats for data transfer RPC operations. New
lines are added that look like:

file-rd : n=55794 nb=10252282150
file-wr : n=9789 nb=3247763645

where n= indicates the number of ops completed and nb= indicates the number
of bytes successfully transferred. file-rd is the counts for read/fetch
operations and file-wr the counts for write/store operations.

Note that directory and symlink downloading are included in the file-rd
stats at the moment.

afs: Fix directory handling

AFS directories are structured blobs that are downloaded just like files
and then parsed by the lookup and readdir code and, as such, are currently
handled in the pagecache like any other file, with the entire directory
content being thrown away each time the directory changes.

However, since the blob is a known structure and since the data version
counter on a directory increases by exactly one for each change committed
to that directory, we can actually edit the directory locally rather than
fetching it from the server after each locally-induced change.

What we can't do, though, is mix data from the server and data from the
client since the server is technically at liberty to rearrange or compress
a directory if it sees fit, provided it updates the data version number
when it does so and breaks the callback (ie. sends a notification).

Further, lookup with lookup-ahead, readdir and, when it arrives, local
editing are likely want to scan the whole of a directory.

So directory handling needs to be improved to maintain the coherency of the
directory blob prior to permitting local directory editing.

To this end:

(1) If any directory page gets discarded, invalidate and reread the entire
directory.

(2) If readpage notes that if when it fetches a single page that the
version number has changed, the entire directory is flagged for
invalidation.

(3) Read as much of the directory in one go as we can.

Note that this removes local caching of directories in fscache for the
moment as we can't pass the pages to fscache_read_or_alloc_pages() since
page->lru is in use by the LRU.

afs: Make afs_write_begin() avoid writing to a page that's being stored

Make afs_write_begin() wait for a page that's marked PG_writeback because:

(1) We need to avoid interference with the data being stored so that the
data on the server ends up in a defined state.

(2) page->private is used to track the window of dirty data within a page,
but it's also used by the storage code to track what's being written,
being cleared by the completion notification. Ownership can't be
relinquished by the storage code until completion because it a store
fails, the data must be remarked dirty.

Tracing shows something like the following (edited):

mm, pagevec: remove cold parameter for pagevecs

Every pagevec_init user claims the pages being released are hot even in
cases where it is unlikely the pages are hot. As no one cares about the
hotness of pages being released to the allocator, just ditch the
parameter.

No performance impact is expected as the overhead is marginal. The
parameter is removed simply because it is a bit stupid to have a useless
parameter copied everywhere.

afs: Overhaul volume and server record caching and fileserver rotation

The current code assumes that volumes and servers are per-cell and are
never shared, but this is not enforced, and, indeed, public cells do exist
that are aliases of each other. Further, an organisation can, say, set up
a public cell and a private cell with overlapping, but not identical, sets
of servers. The difference is purely in the database attached to the VL
servers.

The current code will malfunction if it sees a server in two cells as it
assumes global address -> server record mappings and that each server is in
just one cell.

Further, each server may have multiple addresses - and may have addresses
of different families (IPv4 and IPv6, say).

To this end, the following structural changes are made:

(1) Server record management is overhauled:

(a) Server records are made independent of cell. The namespace keeps
track of them, volume records have lists of them and each vnode
has a server on which its callback interest currently resides.

(b) The cell record no longer keeps a list of servers known to be in
that cell.

(c) The server records are now kept in a flat list because there's no
single address to sort on.

(d) Server records are now keyed by their UUID within the namespace.

(e) The addresses for a server are obtained with the VL.GetAddrsU
rather than with VL.GetEntryByName, using the server's UUID as a
parameter.

(f) Cached server records are garbage collected after a period of
non-use and are counted out of existence before purging is allowed
to complete. This protects the work functions against rmmod.

(g) The servers list is now in /proc/fs/afs/servers.

(2) Volume record management is overhauled:

(a) An RCU-replaceable server list is introduced. This tracks both
servers and their coresponding callback interests.

(b) The superblock is now keyed on cell record and numeric volume ID.

(c) The volume record is now tied to the superblock which mounts it,
and is activated when mounted and deactivated when unmounted.
This makes it easier to handle the cache cookie without causing a
double-use in fscache.

(d) The volume record is loaded from the VLDB using VL.GetEntryByNameU
to get the server UUID list.

(e) The volume name is updated if it is seen to have changed when the
volume is updated (the update is keyed on the volume ID).

(3) The vlocation record is got rid of and VLDB records are no longer
cached. Sufficient information is stored in the volume record, though
an update to a volume record is now no longer shared between related
volumes (volumes come in bundles of three: R/W, R/O and backup).

and the following procedural changes are made:

(1) The fileserver cursor introduced previously is now fleshed out and
used to iterate over fileservers and their addresses.

(2) Volume status is checked during iteration, and the server list is
replaced if a change is detected.

(3) Server status is checked during iteration, and the address list is
replaced if a change is detected.

(4) The abort code is saved into the address list cursor and -ECONNABORTED
returned in afs_make_call() if a remote abort happened rather than
translating the abort into an error message. This allows actions to
be taken depending on the abort code more easily.

(a) If a VMOVED abort is seen then this is handled by rechecking the
volume and restarting the iteration.

(b) If a VBUSY, VRESTARTING or VSALVAGING abort is seen then this is
handled by sleeping for a short period and retrying and/or trying
other servers that might serve that volume. A message is also
displayed once until the condition has cleared.

(c) If a VOFFLINE abort is seen, then this is handled as VBUSY for the
moment.

(d) If a VNOVOL abort is seen, the volume is rechecked in the VLDB to
see if it has been deleted; if not, the fileserver is probably
indicating that the volume couldn't be attached and needs
salvaging.

(e) If statfs() sees one of these aborts, it does not sleep, but
rather returns an error, so as not to block the umount program.

(5) The fileserver iteration functions in vnode.c are now merged into
their callers and more heavily macroised around the cursor. vnode.c
is removed.

(6) Operations on a particular vnode are serialised on that vnode because
the server will lock that vnode whilst it operates on it, so a second
op sent will just have to wait.

(7) Fileservers are probed with FS.GetCapabilities before being used.
This is where service upgrade will be done.

(8) A callback interest on a fileserver is set up before an FS operation
is performed and passed through to afs_make_call() so that it can be
set on the vnode if the operation returns a callback. The callback
interest is passed through to afs_iget() also so that it can be set
there too.

In general, record updating is done on an as-needed basis when we try to
access servers, volumes or vnodes rather than offloading it to work items
and special threads.

Notes:

(1) Pre AFS-3.4 servers are no longer supported, though this can be added
back if necessary (AFS-3.4 was released in 1998).

(2) VBUSY is retried forever for the moment at intervals of 1s.

(3) /proc/fs/afs/<cell>/servers no longer exists.

fs: convert a pile of fsync routines to errseq_t based reporting

This patch converts most of the in-kernel filesystems that do writeback
out of the pagecache to report errors using the errseq_t-based
infrastructure that was recently added. This allows them to report
errors once for each open file description.

Most filesystems have a fairly straightforward fsync operation. They
call filemap_write_and_wait_range to write back all of the data and
wait on it, and then (sometimes) sync out the metadata.

For those filesystems this is a straightforward conversion from calling
filemap_write_and_wait_range in their fsync operation to calling
file_write_and_wait_range.

(1) If a writeback has been partially flushed, then if we try and kill the
pages it contains, some of them may no longer be undergoing writeback
and end_page_writeback() will assert.

Fix this by checking to see whether the page in question is actually
undergoing writeback before ending that writeback.

(2) The loop that scans for pages to kill doesn't increase the first page
index, and so the loop may not terminate, but it will try to process
the same pages over and over again.

Fix this by increasing the first page index to one after the last page
we processed.

afs_fill_page() loads the page it wants to fill into the afs_read request
without incrementing its refcount - but then calls afs_put_read() to clean
up afterwards, which then releases a ref on the page.

Fix this by getting a ref on the page before calling
afs_vnode_fetch_data().

This causes sync after a write to hang in afs_writepages_region() because
find_get_pages_tag() gets confused and doesn't return.

remove lots of IS_ERR_VALUE abuses

Most users of IS_ERR_VALUE() in the kernel are wrong, as they
pass an 'int' into a function that takes an 'unsigned long'
argument. This happens to work because the type is sign-extended
on 64-bit architectures before it gets converted into an
unsigned type.

However, anything that passes an 'unsigned short' or 'unsigned int'
argument into IS_ERR_VALUE() is guaranteed to be broken, as are
8-bit integers and types that are wider than 'unsigned long'.

Andrzej Hajda has already fixed a lot of the worst abusers that
were causing actual bugs, but it would be nice to prevent any
users that are not passing 'unsigned long' arguments.

This patch changes all users of IS_ERR_VALUE() that I could find
on 32-bit ARM randconfig builds and x86 allmodconfig. For the
moment, this doesn't change the definition of IS_ERR_VALUE()
because there are probably still architecture specific users
elsewhere.

Almost all the warnings I got are for files that are better off
using 'if (err)' or 'if (err < 0)'.
The only legitimate user I could find that we get a warning for
is the (32-bit only) freescale fman driver, so I did not remove
the IS_ERR_VALUE() there but changed the type to 'unsigned long'.
For 9pfs, I just worked around one user whose calling conventions
are so obscure that I did not dare change the behavior.

I was using this definition for testing:

unlikely((unsigned long long)(x) >= (unsigned long long)(typeof(x))-MAX_ERRNO))

which ends up making all 16-bit or wider types work correctly with
the most plausible interpretation of what IS_ERR_VALUE() was supposed
to return according to its users, but also causes a compile-time
warning for any users that do not pass an 'unsigned long' argument.

I suggested this approach earlier this year, but back then we ended
up deciding to just fix the users that are obviously broken. After
the initial warning that caused me to get involved in the discussion
(fs/gfs2/dir.c) showed up again in the mainline kernel, Linus
asked me to send the whole thing again.

mm, fs: get rid of PAGE_CACHE_* and page_cache_{get,release} macros

PAGE_CACHE_{SIZE,SHIFT,MASK,ALIGN} macros were introduced *long* time
ago with promise that one day it will be possible to implement page
cache with bigger chunks than PAGE_SIZE.

This promise never materialized. And unlikely will.

We have many places where PAGE_CACHE_SIZE assumed to be equal to
PAGE_SIZE. And it's constant source of confusion on whether
PAGE_CACHE_* or PAGE_* constant should be used in a particular case,
especially on the border between fs and mm.

Global switching to PAGE_CACHE_SIZE != PAGE_SIZE would cause to much
breakage to be doable.

Let's stop pretending that pages in page cache are special. They are
not.

The changes are pretty straight-forward:

- <foo> << (PAGE_CACHE_SHIFT - PAGE_SHIFT) -> <foo>;

- <foo> >> (PAGE_CACHE_SHIFT - PAGE_SHIFT) -> <foo>;

- PAGE_CACHE_{SIZE,SHIFT,MASK,ALIGN} -> PAGE_{SIZE,SHIFT,MASK,ALIGN};

- page_cache_get() -> get_page();

- page_cache_release() -> put_page();

This patch contains automated changes generated with coccinelle using
script below. For some reason, coccinelle doesn't patch header files.
I've called spatch for them manually.

The only adjustment after coccinelle is revert of changes to

fs: push i_mutex and filemap_write_and_wait down into ->fsync() handlers

Btrfs needs to be able to control how filemap_write_and_wait_range() is called
in fsync to make it less of a painful operation, so push down taking i_mutex and
the calling of filemap_write_and_wait() down into the ->fsync() handlers. Some
file systems can drop taking the i_mutex altogether it seems, like ext3 and
ocfs2. For correctness sake I just pushed everything down in all cases to make
sure that we keep the current behavior the same for everybody, and then each
individual fs maintainer can make up their mind about what to do from there.

afs: afs_fill_page reads too much, or wrong data

afs_fill_page should read the page that is about to be written but
the current implementation has a number of issues. If we aren't
extending the file we always read PAGE_CACHE_SIZE at offset 0. If we
are extending the file we try to read the entire file.

Change afs_fill_page to read PAGE_CACHE_SIZE at the right offset,
clamped to i_size.

While here, avoid calling afs_fill_page when we are doing a
PAGE_CACHE_SIZE write.

writeback: remove nonblocking/encountered_congestion references

This removes more dead code that was somehow missed by commit 0d99519efef
(writeback: remove unused nonblocking and congestion checks). There are
no behavior change except for the removal of two entries from one of the
ext4 tracing interface.

The nonblocking checks in ->writepages are no longer used because the
flusher now prefer to block on get_request_wait() than to skip inodes on
IO congestion. The latter will lead to more seeky IO.

The nonblocking checks in ->writepage are no longer used because it's
redundant with the WB_SYNC_NONE check.

We no long set ->nonblocking in VM page out and page migration, because
a) it's effectively redundant with WB_SYNC_NONE in current code
b) it's old semantic of "Don't get stuck on request queues" is mis-behavior:
that would skip some dirty inodes on congestion and page out others, which
is unfair in terms of LRU age.

make sure data is on disk before calling ->write_inode

Similar to the fsync issue fixed a while ago in commit
2daea67e966dc0c42067ebea015ddac6834cef88 we need to write for data to
actually hit the disk before writing out the metadata to guarantee
data integrity for filesystems that modify the inode in the data I/O
completion path. Currently XFS and NFS handle this manually, and AFS
has a write_inode method that does nothing but waiting for data, while
others are possibly missing out on this.

Fortunately this change has a lot less impact than the fsync change
as none of the write_inode methods starts data writeout of any form
by itself.

vfs: Implement proper O_SYNC semantics

While Linux provided an O_SYNC flag basically since day 1, it took until
Linux 2.4.0-test12pre2 to actually get it implemented for filesystems,
since that day we had generic_osync_around with only minor changes and the
great "For now, when the user asks for O_SYNC, we'll actually give
O_DSYNC" comment. This patch intends to actually give us real O_SYNC
semantics in addition to the O_DSYNC semantics. After Jan's O_SYNC
patches which are required before this patch it's actually surprisingly
simple, we just need to figure out when to set the datasync flag to
vfs_fsync_range and when not.

This patch renames the existing O_SYNC flag to O_DSYNC while keeping it's
numerical value to keep binary compatibility, and adds a new real O_SYNC
flag. To guarantee backwards compatiblity it is defined as expanding to
both the O_DSYNC and the new additional binary flag (__O_SYNC) to make
sure we are backwards-compatible when compiled against the new headers.

This also means that all places that don't care about the differences can
just check O_DSYNC and get the right behaviour for O_SYNC, too - only
places that actuall care need to check __O_SYNC in addition. Drivers and
network filesystems have been updated in a fail safe way to always do the
full sync magic if O_DSYNC is set. The few places setting O_SYNC for
lower layers are kept that way for now to stay failsafe.

We enforce that O_DSYNC is set when __O_SYNC is set early in the open path
to make sure we always get these sane options.

Note that parisc really screwed up their headers as they already define a
O_DSYNC that has always been a no-op. We try to repair it by using it for
the new O_DSYNC and redefinining O_SYNC to send both the traditional
O_SYNC numerical value _and_ the O_DSYNC one.

fs: symlink write_begin allocation context fix

With the write_begin/write_end aops, page_symlink was broken because it
could no longer pass a GFP_NOFS type mask into the point where the
allocations happened. They are done in write_begin, which would always
assume that the filesystem can be entered from reclaim. This bug could
cause filesystem deadlocks.

The funny thing with having a gfp_t mask there is that it doesn't really
allow the caller to arbitrarily tinker with the context in which it can be
called. It couldn't ever be GFP_ATOMIC, for example, because it needs to
take the page lock. The only thing any callers care about is __GFP_FS
anyway, so turn that into a single flag.

Add a new flag for write_begin, AOP_FLAG_NOFS. Filesystems can now act on
this flag in their write_begin function. Change __grab_cache_page to
accept a nofs argument as well, to honour that flag (while we're there,
change the name to grab_cache_page_write_begin which is more instructive
and does away with random leading underscores).

This is really a more flexible way to go in the end anyway -- if a
filesystem happens to want any extra allocations aside from the pagecache
ones in ints write_begin function, it may now use GFP_KERNEL (rather than
GFP_NOFS) for common case allocations (eg. ocfs2_alloc_write_ctxt, for a
random example).

(1) The afs_send_pages() outer loop must do an extra iteration if it ends
with 'first == last' because 'last' is inclusive in the page set
otherwise it fails to send the last page and complete the RxRPC op under
some circumstances.

(2) Similarly, the outer loop in afs_pages_written_back() must also do an
extra iteration if it ends with 'first == last', otherwise it fails to
clear PG_writeback on the last page under some circumstances.

AFS: implement basic file write support

Implement support for writing to regular AFS files, including:

(1) write

(2) truncate

(3) fsync, fdatasync

(4) chmod, chown, chgrp, utime.

AFS writeback attempts to batch writes into as chunks as large as it can manage
up to the point that it writes back 65535 pages in one chunk or it meets a
locked page.

Furthermore, if a page has been written to using a particular key, then should
another write to that page use some other key, the first write will be flushed
before the second is allowed to take place. If the first write fails due to a
security error, then the page will be scrapped and reread before the second
write takes place.

If a page is dirty and the callback on it is broken by the server, then the
dirty data is not discarded (same behaviour as NFS).

Shared-writable mappings are not supported by this patch.
