scsi: ips: Do not try to abort command from host reset

The code for aborting an outstanding command is a copy of the functionality
from command abort. As we already have called this function once we reach
host reset there's no point in trying to do so again.

scsi: ips: Replace kmap_atomic() with kmap_local_page()

kmap_atomic() is deprecated in favor of kmap_local_page(). Therefore,
replace kmap_atomic() with kmap_local_page() in ips_is_passthru(). In the
meantime remove an unnecessary comment, align code, and remove spaces.

kmap_atomic() is implemented like a kmap_local_page() which also disables
page-faults and preemption (the latter only for !PREEMPT_RT kernels). The
code within the mapping/unmapping in ips_is_passthru() is already in atomic
context because of a call to local_irq_save() and kmap_local_page() can be
called in atomic context too (including interrupts).

Therefore, a mere replacement of the old API with the new one is all it is
required (i.e., there is no need to explicitly add any calls to
pagefault_disable() and/or preempt_disable()).

The following two header files have the same file name: include/scsi/scsi.h
and drivers/scsi/scsi.h. This is confusing. Remove the latter since the
following note was added in drivers/scsi/scsi.h in 2004:

"NOTE: this file only contains compatibility glue for old drivers. All
these wrappers will be removed sooner or later. For new code please use
the interfaces declared in the headers in include/scsi/"

The DEF_SCSI_QCMD() macro passes the addresses of the SCSI host lock and
also that of the scsi_done function to the queuecommand_lck() function
implementations. Remove the 'scsi_done' argument since its address is
now a constant and instead call 'scsi_done' directly from inside the
queuecommand_lck() functions.

In preparation for FORTIFY_SOURCE performing compile-time and run-time
field bounds checking for memcpy() avoid intentionally reading across
neighboring array fields.

scb->scsi_cmd->sense_buffer is 96 bytes:

treewide: remove editor modelines and cruft

The section "19) Editor modelines and other cruft" in
Documentation/process/coding-style.rst clearly says, "Do not include any
of these in source files."

I recently receive a patch to explicitly add a new one.

Let's do treewide cleanups, otherwise some people follow the existing code
and attempt to upstream their favoriate editor setups.

It is even nicer if scripts/checkpatch.pl can check it.

If we like to impose coding style in an editor-independent manner, I think
editorconfig (patch [1]) is a saner solution.

scsi: remove the use_clustering flag

The same effects can be achieved by setting the dma_boundary to
PAGE_SIZE - 1 and the max_segment_size to PAGE_SIZE, so shift those
settings into the drivers. Note that in many cases the setting might
be bogus, but this keeps the status quo.

Most SCSI drivers want to enable "clustering", that is merging of
segments so that they might span more than a single page. Remove the
ENABLE_CLUSTERING define, and require drivers to explicitly set
DISABLE_CLUSTERING to disable this feature.

scsi: ips: fix firmware timestamps for 32-bit

do_gettimeofday() is deprecated since it will stop working in 2038 on
32-bit platforms, leading to incorrect times passed to the firmware.
On 64-bit platforms the current code appears to be fine, as the
calculation passes an 8-bit century number into the firmware that can
represent times long in the future (possibly until 25599).

Using ktime_get_real_seconds() to get a 64-bit seconds value and
time64_to_tm() to convert it into the firmware format greatly simplifies
the ips timekeeping code, makes 32-bit and 64-bit behave the same way
here, and gets us closer to removing the deprecated interfaces.

sched/wait: Rename wait_queue_t => wait_queue_entry_t

Rename:

wait_queue_t => wait_queue_entry_t

'wait_queue_t' was always a slight misnomer: its name implies that it's a "queue",
but in reality it's a queue *entry*. The 'real' queue is the wait queue head,
which had to carry the name.

Start sorting this out by renaming it to 'wait_queue_entry_t'.

This also allows the real structure name 'struct __wait_queue' to
lose its double underscore and become 'struct wait_queue_entry',
which is the more canonical nomenclature for such data types.

non-x86 builds want the #warning in the IPS code about compiling on the wrong
architecture removed because it keeps triggering on their platforms build
farms. Transform from a compile time warning into a runtime one with taint to
preserve the original intent of the authors.

scsi: don't set tagging state from scsi_adjust_queue_depth

Remove the tagged argument from scsi_adjust_queue_depth, and just let it
handle the queue depth. For most drivers those two are fairly separate,
given that most modern drivers don't care about the SCSI "tagged" status
of a command at all, and many old drivers allow queuing of multiple
untagged commands in the driver.

Instead we start out with the ->simple_tags flag set before calling
->slave_configure, which is how all drivers actually looking at
->simple_tags except for one worke anyway. The one other case looks
broken, but I've kept the behavior as-is for now.

Except for that we only change ->simple_tags from the ->change_queue_type,
and when rejecting a tag message in a single driver, so keeping this
churn out of scsi_adjust_queue_depth is a clear win.

Now that the usage of scsi_adjust_queue_depth is more obvious we can
also remove all the trivial instances in ->slave_alloc or ->slave_configure
that just set it to the cmd_per_lun default.

scsi: replace strnicmp with strncasecmp

The kernel used to contain two functions for length-delimited,
case-insensitive string comparison, strnicmp with correct semantics and
a slightly buggy strncasecmp. The latter is the POSIX name, so strnicmp
was renamed to strncasecmp, and strnicmp made into a wrapper for the new
strncasecmp to avoid breaking existing users.

To allow the compat wrapper strnicmp to be removed at some point in the
future, and to avoid the extra indirection cost, do
s/strnicmp/strncasecmp/g.

[SCSI] Disable WRITE SAME for RAID and virtual host adapter drivers

Some host adapters do not pass commands through to the target disk
directly. Instead they provide an emulated target which may or may not
accurately report its capabilities. In some cases the physical device
characteristics are reported even when the host adapter is processing
commands on the device's behalf. This can lead to adapter firmware hangs
or excessive I/O errors.

This patch disables WRITE SAME for devices connected to host adapters
that provide an emulated target. Driver writers can disable WRITE SAME
by setting the no_write_same flag in the host adapter template.

SCSI host lock push-down

Move the mid-layer's ->queuecommand() invocation from being locked
with the host lock to being unlocked to facilitate speeding up the
critical path for drivers who don't need this lock taken anyway.

The patch below presents a simple SCSI host lock push-down as an
equivalent transformation. No locking or other behavior should change
with this patch. All existing bugs and locking orders are preserved.

Additionally, add one parameter to queuecommand,
struct Scsi_Host *
and remove one parameter from queuecommand,
void (*done)(struct scsi_cmnd *)

Scsi_Host* is a convenient pointer that most host drivers need anyway,
and 'done' is redundant to struct scsi_cmnd->scsi_done.

Minimal code disturbance was attempted with this change. Most drivers
needed only two one-line modifications for their host lock push-down.

[SCSI] Remove SUGGEST flags

block: unify request timeout handling

Right now SCSI and others do their own command timeout handling.
Move those bits to the block layer.

Instead of having a timer per command, we try to be a bit more clever
and simply have one per-queue. This avoids the overhead of having to
tear down and setup a timer for each command, so it will result in a lot
less timer fiddling.

I overlooked ips_scmd_buf_write and ips_scmd_buf_read when I converted
ips to use the data buffer accessors.

ips is unlikely to use sg chaining (especially in this path) since a)
this path is used only for non I/O commands (with little data
transfer), b) ips's sg_tablesize is set to just 17.

LLDs don't need to zero out scsi_cmnd::sense_buffer in queuecommand
since scsi-ml does. This is a preparation of the future changes to
allocate the sense_buffer only when necessary.

Many LLDs zero out the sense_buffer before touching it on the error
case. This patch lets them alone for now because new APIs for them
would be added later on.

PCI: Change all drivers to use pci_device->revision

Instead of all drivers reading pci config space to get the revision
ID, they can now use the pci_device->revision member.

This exposes some issues where drivers where reading a word or a dword
for the revision number, and adding useless error-handling around the
read. Some drivers even just read it for no purpose of all.

In devices where the revision ID is being copied over and used in what
appears to be the equivalent of hotpath, I have left the copy code
and the cached copy as not to influence the driver's performance.

Compile tested with make all{yes,mod}config on x86_64 and i386.

IRQ: Maintain regs pointer globally rather than passing to IRQ handlers

Maintain a per-CPU global "struct pt_regs *" variable which can be used instead
of passing regs around manually through all ~1800 interrupt handlers in the
Linux kernel.

The regs pointer is used in few places, but it potentially costs both stack
space and code to pass it around. On the FRV arch, removing the regs parameter
from all the genirq function results in a 20% speed up of the IRQ exit path
(ie: from leaving timer_interrupt() to leaving do_IRQ()).

Where appropriate, an arch may override the generic storage facility and do
something different with the variable. On FRV, for instance, the address is
maintained in GR28 at all times inside the kernel as part of general exception
handling.

Having looked over the code, it appears that the parameter may be handed down
through up to twenty or so layers of functions. Consider a USB character
device attached to a USB hub, attached to a USB controller that posts its
interrupts through a cascaded auxiliary interrupt controller. A character
device driver may want to pass regs to the sysrq handler through the input
layer which adds another few layers of parameter passing.

I've build this code with allyesconfig for x86_64 and i386. I've runtested the
main part of the code on FRV and i386, though I can't test most of the drivers.
I've also done partial conversion for powerpc and MIPS - these at least compile
with minimal configurations.

This will affect all archs. Mostly the changes should be relatively easy.
Take do_IRQ(), store the regs pointer at the beginning, saving the old one:

And put the old one back at the end:

set_irq_regs(old_regs);

Don't pass regs through to generic_handle_irq() or __do_IRQ().

In timer_interrupt(), this sort of change will be necessary:

- update_process_times(user_mode(regs));
- profile_tick(CPU_PROFILING, regs);
+ update_process_times(user_mode(get_irq_regs()));
+ profile_tick(CPU_PROFILING);

I'd like to move update_process_times()'s use of get_irq_regs() into itself,
except that i386, alone of the archs, uses something other than user_mode().

Some notes on the interrupt handling in the drivers:

(*) input_dev() is now gone entirely. The regs pointer is no longer stored in
the input_dev struct.

(*) finish_unlinks() in drivers/usb/host/ohci-q.c needs checking. It does
something different depending on whether it's been supplied with a regs
pointer or not.

(*) Various IRQ handler function pointers have been moved to type
irq_handler_t.

[SCSI] fix up request buffer reference in various scsi drivers

Various scsi drivers use scsi_cmnd.buffer and scsi_cmnd.bufflen in their
queuecommand functions. Those fields are internal storage for the
midlayer only and are used to restore the original payload after
request_buffer and request_bufflen have been overwritten for EH. Using
the buffer and bufflen fields means they do very broken things in error
handling.

[SCSI] don't call ips_eh_reset in ips_queue to avoid deadlock

When the locking was changed in the eh code ips_eh_reset was changed
so that it was a wraper around __ips_eh_reset and all ips_eh_reset
does is grab the host lock and then calls __ips_eh_reset.

In the queuecommand, ips_queue is called with the host_lock held so if
it calls ips_eh_reset we will have a problem. This patch just has
ips_queue call __ips_eh_reset.

Patch is only compile tested. I do not have the HW.

[SCSI] ServeRAID: prevent seeing DADSI devices

A critical thing the ServeRAID driver MUST do is hide the physical DASDI
devices from the OS. It does this by intercepting the INQUIRY commands.

In recent 2.6.15 testing, I discovered this to be failing.

The cause was the driver assuming that the INQUIRY response data was in a
simple single buffer, when it was actually a 1 element scatter gather list.

This patch makes ips always look at the correct data when examining an
INQUIRY response.

Resetting the adapter causes the ServeRAID driver to exceed
the max time allowed by the softlock watchdog. Resetting the
hardware can easily require 30 or more seconds. To avoid the

"BUG: soft lockup detected on CPU#0!"

result, this patch replaces the mdelay() calls in the
initialization/reset routines with msleep().

[SCSI] ips: Fix initialization bug with kdump

If I/O is active on the adapter, and an unexpected interrupt is pending
during initialization, the driver blows it's brains out. Since the driver
didn't initiate the I/O, the data in it's internal tables will contain NULL
pointers.

When this condition is detected, a "flush cache and reset" is performed.
The flush cache allows any pending "lazy writes" that the adapter is
processing to complete ( a "must have" for a RAID adapter ) and the reset
puts the adapter back into a known, good state.

scsi_cmnd->serial_number_at_timeout doesn't serve any purpose
anymore. All serial_number == serial_number_at_timeout tests
are always true in abort callbacks. Kill the field. Also, as
->pid always equals ->serial_number and ->serial_number

[PATCH] scsi: remove meaningless scsi_cmnd->serial_number_at_timeout field

scsi_cmnd->serial_number_at_timeout doesn't serve any purpose
anymore. All serial_number == serial_number_at_timeout tests
are always true in abort callbacks. Kill the field. Also, as
->pid always equals ->serial_number and ->serial_number
doesn't have any special meaning anymore, update comments
above ->serial_number accordingly. Once we remove all uses of
this field from all lldd's, this field should go.

Linux-2.6.12-rc2

Initial git repository build. I'm not bothering with the full history,
even though we have it. We can create a separate "historical" git
archive of that later if we want to, and in the meantime it's about
3.2GB when imported into git - space that would just make the early
git days unnecessarily complicated, when we don't have a lot of good
infrastructure for it.

Let it rip!
