ptp: convert remaining drivers to adjfine interface

Convert all remaining drivers that still use .adjfreq to the newer .adjfine
implementation. These drivers are not straightforward, as they use
non-standard methods of programming their hardware. They are all converted
to use scaled_ppm_to_ppb to get the parts per billion value that their
logic depends on.

cxgb4: fix large delays in PTP synchronization

Fetching PTP sync information from mailbox is slow and can take
up to 10 milliseconds. Reduce this unnecessary delay by directly
reading the information from the corresponding registers.

cxgb4/ptp: pass the sign of offset delta in FW CMD

cxgb4_ptp_fineadjtime() doesn't pass the signedness of offset delta
in FW_PTP_CMD. Fix it by passing correct sign.

cxgb4: ptp_clock_register() returns error pointers

Check ptp_clock_register() return not only for NULL but
also for error pointers, and also nullify adapter->ptp_clock
if ptp_clock_register() fails.

cxgb4: Add PTP Hardware Clock (PHC) support

Add PTP IEEE-1588 support and make it accessible via PHC subsystem.
The functionality is enabled for T5/T6 adapters. Driver interfaces with
Firmware to program and adjust the clock offset.
