block: Change the return type of blk_mq_map_queues() into void

Since blk_mq_map_queues() and the .map_queues() callbacks always return 0,
change their return type into void. Most callers ignore the returned value
anyway.

blk-mq: initial support for multiple queue maps

Add a queue offset to the tag map. This enables users to map
iteratively, for each queue map type they support.

Bump maximum number of supported maps to 2, we're now fully
able to support more than 1 map.

blk-mq: code clean-up by adding an API to clear set->mq_map

set->mq_map is now currently cleared if something goes wrong when
establishing a queue map in blk-mq-pci.c. It's also cleared before
updating a queue map in blk_mq_update_queue_map().

This patch provides an API to clear set->mq_map to make it clear.

blk-mq: Allow PCI vector offset for mapping queues

The PCI interrupt vectors intended to be associated with a queue may
not start at 0; a driver may allocate pre_vectors for special use. This
patch adds an offset parameter so blk-mq may find the intended affinity
mask and updates all drivers using this API accordingly.

blk-mq-pci: add a fallback when pci_irq_get_affinity returns NULL

While pci_irq_get_affinity should never fail for SMP kernel that
implement the affinity mapping, it will always return NULL in the
UP case, so provide a fallback mapping of all queues to CPU 0 in
that case.
