powerpc/64: Adjust order in pcibios_init() The pcibios_init() function for PowerPC 64 currently calls pci_bus_add_devices() before pcibios_resource_survey(). This means that at boot time, when the pcibios_bus_add_device() hooks are called by pci_bus_add_devices(), device resources have not been allocated and they are unable to perform EEH setup, so a separate pass is needed. This patch adjusts that order so that it will become possible to consolidate the EEH setup work into a single location. The only functional change is to execute pcibios_resource_survey() (excepting ppc_md.pcibios_fixup(), see below) before pci_bus_add_devices() instead of after it. Because pcibios_scan_phb() and pci_bus_add_devices() are called together in a loop, this must be broken into one loop for each call. Then the call to pcibios_resource_survey() is moved up in between them. This changes the ordering but because pcibios_resource_survey() also calls ppc_md.pcibios_fixup(), that call is extracted out into pcibios_init() to where pcibios_resource_survey() was, so that it is not moved. The only other caller of pcibios_resource_survey() is the PowerPC 32 version of pcibios_init(), and therefore, that is modified to call ppc_md.pcibios_fixup() right after pcibios_resource_survey() so that there is no functional change there at all. The re-arrangement will cause very few side-effects because at this stage in the boot, pci_bus_add_devices() does very little: - pci_create_sysfs_dev_files() does nothing (no sysfs yet) - pci_proc_attach_device() does nothing (no proc yet) - device_attach() does nothing (no drivers yet) This leaves only the pci_final_fixup calls, D3 support, and marking the device as added. Of those, only the pci_final_fixup calls have the potential to be affected by resource allocation. The only pci_final_fixup handlers that touch resources seem to be one for x86 (pci_amd_enable_64bit_bar()), and a PowerPC 32 platform driver (quirk_final_uli1575()), neither of which use this pcibios_init() function. Even if they did, it would almost certainly be a bug, under the current ordering, to rely on or make changes to resources before they were allocated. powerpc/64s: Replace CONFIG_PPC_STD_MMU_64 with CONFIG_PPC_BOOK3S_64 CONFIG_PPC_STD_MMU_64 indicates support for the "standard" powerpc MMU on 64-bit CPUs. The "standard" MMU refers to the hash page table MMU found in "server" processors, from IBM mainly. Currently CONFIG_PPC_STD_MMU_64 is == CONFIG_PPC_BOOK3S_64. While it's annoying to have two symbols that always have the same value, it's not quite annoying enough to bother removing one. However with the arrival of Power9, we now have the situation where CONFIG_PPC_STD_MMU_64 is enabled, but the kernel is running using the Radix MMU - *not* the "standard" MMU. So it is now actively confusing to use it, because it implies that code is disabled or inactive when the Radix MMU is in use, however that is not necessarily true. So s/CONFIG_PPC_STD_MMU_64/CONFIG_PPC_BOOK3S_64/, and do some minor formatting updates of some of the affected lines. This will be a pain for backports, but c'est la vie. powerpc: Initialise pci_io_base as early as possible Commit d6a9996e84ac ("powerpc/mm: vmalloc abstraction in preparation for radix") turned kernel memory and IO addresses from #defined constants to variables initialised at runtime. On PA6T (pasemi) systems the setup_arch() machine call initialises the onboard PCI-e root-ports, and uses pci_io_base to do this, which is now before its value has been set, resulting in a panic early in boot before console IO is initialised. Move the pci_io_base initialisation to the same place as vmalloc ranges are set (hash__early_init_mmu()/radix__early_init_mmu()) - this is the earliest possible place we can initialise it. powerpc/pci: Support per-aperture memory offset The PCI core supports an offset per aperture nowadays but our arch code still has a single offset per host bridge representing the difference betwen CPU memory addresses and PCI MMIO addresses. This is a problem as new machines and hypervisor versions are coming out where the 64-bit windows will have a different offset (basically mapped 1:1) from the 32-bit windows. This fixes it by using separate offsets. In the long run, we probably want to get rid of that intermediary struct pci_controller and have those directly stored into the pci_host_bridge as they are parsed but this will be a more invasive change. powerpc: Fix PCI ROM access A couple of issues crept in since about 2.6.27 related to accessing PCI device ROMs on various powerpc machines. First, historically, we don't allocate the ROM resource in the resource tree. I'm not entirely certain of why, I susepct they often contained garbage on x86 but it's hard to tell. This causes the current generic code to always call pci_assign_resource() when trying to access the said ROM from sysfs, which will try to re-assign some new address regardless of what the ROM BAR was already set to at boot time. This can be a problem on hypervisor platforms like pSeries where we aren't supposed to move PCI devices around (and in fact probably can't). Second, our code that generates the PCI tree from the OF device-tree (instead of doing config space probing) which we mostly use on pseries at the moment, didn't set the (new) flag IORESOURCE_SIZEALIGN on any resource. That means that any attempt at re-assigning such a resource with pci_assign_resource() would fail due to resource_alignment() returning 0. This fixes this by doing these two things: - The code that calculates resource flags based on the OF device-node is improved to set IORESOURCE_SIZEALIGN on any valid BAR, and while at it also set IORESOURCE_READONLY for ROMs since we were lacking that too - We now allocate ROM resources as part of the resource tree. However to limit the chances of nasty conflicts due to busted firmwares, we only do it on the second pass of our two-passes allocation scheme, so that all valid and enabled BARs get precedence. This brings pSeries back the ability to access PCI ROMs via sysfs (and thus initialize various video cards from X etc...). Signed-off-by: Benjamin Herrenschmidt <benh@kernel.crashing.org>

powerpc: Fix pciconfig_iobase system call on PCI-Express powermac X has been failing to start on my quad G5 powermac since commit 1fd0f52583a85b21a394201b007bc1ee104b235d ("powerpc: Fix domain numbers in /proc on 64-bit") went in. The reason is that the change allows X to see the PCI-PCI bridge above the video card (previously it was obscured by the fact that there were two "00" directories in /proc/bus/pci), and the pciconfig_iobase system call on the bridge is failing because of a hack that we have to return information about the AGP bus when X asks about bus 0. This machine doesn't have an AGP bus (it has PCI Express) and so the pciconfig_iobase call is returning -1, which ultimately causes X to fail to start. This fixes it by checking that we have an AGP bridge before redirecting the pciconfig_iobase call to return information about the AGP bus. With this, X starts successfully both on a quad G5 with PCI Express and on an older dual G5 with AGP. powerpc/pci: Split pcibios_fixup_bus() into bus setup and device setup Currently, our PCI code uses the pcibios_fixup_bus() callback, which is called by the generic code when probing PCI buses, for two different things. One is to set up things related to the bus itself, such as reading bridge resources for P2P bridges, fixing them up, or setting up the iommu's associated with bridges on some platforms. The other is some setup for each individual device under that bridge, mostly setting up DMA mappings and interrupts. The problem is that this approach doesn't work well with PCI hotplug when an existing bus is re-probed for new children. We fix this problem by splitting pcibios_fixup_bus into two routines: pcibios_setup_bus_self() is now called to setup the bus itself pcibios_setup_bus_devices() is now called to setup devices pcibios_fixup_bus() is then modified to call these two after reading the bridge bases, and the OF based PCI probe is modified to avoid calling into the first one when rescanning an existing bridge. powerpc/pci: Use common PHB resource hookup The 32-bit and 64-bit powerpc PCI code used to set up the resource pointers of the root bus of a given PHB in completely different places. This unifies this in large part, by making 32-bit use a routine very similar to what 64-bit does when initially scanning the PCI busses. The actual setup of the PHB resources itself is then moved to a common function in pci-common.c. This should cause no functional change on 64-bit. On 32-bit, the effect is that the PHB resources are going to be setup a bit earlier, instead of being setup from pcibios_fixup_bus(). powerpc: Merge 32 and 64-bit dma code We essentially adopt the 64-bit dma code, with some changes to support 32-bit systems, including HIGHMEM. dma functions on 32-bit are now invoked via accessor functions which call the correct op for a device based on archdata dma_ops. If there is no archdata dma_ops, this defaults to dma_direct_ops. In addition, the dma_map/unmap_page functions are added to dma_ops because we can't just fall back on map/unmap_single when HIGHMEM is enabled. In the case of dma_direct_*, we stop using map/unmap_single and just use the page version - this saves a lot of ugly ifdeffing. We leave map/unmap_single in the dma_ops definition, though, because they are needed by the iommu code, which does not implement map/unmap_page. Ideally, going forward, we will completely eliminate map/unmap_single and just have map/unmap_page, if it's workable for 64-bit. [POWERPC] fix iSeries PCI resource management The way iSeries manages PCI IO and Memory resources is a bit strange and is based on overriding the content of those resources with home cooked ones afterward. This changes it a bit to better integrate with the new resource handling so that the "virtual" tokens that iSeries replaces resources with are done from the proper per-device fixup hook, and bridge resources are set to enclose that token space. This fixes various things such as the output of /proc/iomem & ioports, among others. This also fixes up various boot messages as well. [POWERPC] Merge PCI resource allocation & assignment The 32 bits PCI code now uses the generic code for assigning unassigned resources and an algorithm similar to x86 for claiming existing ones. This works far better than the 64 bits code which basically can only claim existing ones (pci_probe_only=1) or would fall apart completely. This merges them so that the new 32 bits implementation is used for both. 64 bits now gets the new PCI flags for controlling the behaviour, though the old pci_probe_only global is still there for now to be cleared if you want to. I kept a pcibios_claim_one_bus() function mostly based on the old 64 bits code for use by the DLPAR hotplug. This will have to be cleaned up, thought I hope it will work in the meantime. [POWERPC] Implement pci_set_dma_mask() in terms of the dma_ops PowerPC currently doesn't implement pci_set_dma_mask(), which means drivers calling it will get the generic version in drivers/pci/pci.c. The powerpc dma mapping ops include a dma_set_mask() hook, which luckily is not implemented by anyone - so there is no bug in the fact that the hook is currently never called. However in future we'll add implementation(s) of dma_set_mask(), and so we need pci_set_dma_mask() to call the hook. To save adding a hook to the dma mapping ops, pci-set_consistent_dma_mask() simply calls the dma_set_mask() hook and then copies the new mask into dev.coherenet_dma_mask. [POWERPC] Rewrite IO allocation & mapping on powerpc64 This rewrites pretty much from scratch the handling of MMIO and PIO space allocations on powerpc64. The main goals are: - Get rid of imalloc and use more common code where possible - Simplify the current mess so that PIO space is allocated and mapped in a single place for PCI bridges - Handle allocation constraints of PIO for all bridges including hot plugged ones within the 2GB space reserved for IO ports, so that devices on hotplugged busses will now work with drivers that assume IO ports fit in an int. - Cleanup and separate tracking of the ISA space in the reserved low 64K of IO space. No ISA -> Nothing mapped there. I booted a cell blade with IDE on PIO and MMIO and a dual G5 so far, that's it :-) With this patch, all allocations are done using the code in mm/vmalloc.c, though we use the low level __get_vm_area with explicit start/stop constraints in order to manage separate areas for vmalloc/vmap, ioremap, and PCI IOs. This greatly simplifies a lot of things, as you can see in the diffstat of that patch :-) A new pair of functions pcibios_map/unmap_io_space() now replace all of the previous code that used to manipulate PCI IOs space. The allocation is done at mapping time, which is now called from scan_phb's, just before the devices are probed (instead of after, which is by itself a bug fix). The only other caller is the PCI hotplug code for hot adding PCI-PCI bridges (slots). imalloc is gone, as is the "sub-allocation" thing, but I do beleive that hotplug should still work in the sense that the space allocation is always done by the PHB, but if you unmap a child bus of this PHB (which seems to be possible), then the code should properly tear down all the HPTE mappings for that area of the PHB allocated IO space. I now always reserve the first 64K of IO space for the bridge with the ISA bus on it. I have moved the code for tracking ISA in a separate file which should also make it smarter if we ever are capable of hot unplugging or re-plugging an ISA bridge. This should have a side effect on platforms like powermac where VGA IOs will no longer work. This is done on purpose though as they would have worked semi-randomly before. The idea at this point is to isolate drivers that might need to access those and fix them by providing a proper function to obtain an offset to the legacy IOs of a given bus. [POWERPC] Cope with PCI host bridge I/O window not starting at 0 Currently our code to set up the data structures for a PCI host bridge and create the mapping for its I/O window assumes that the window starts at I/O port 0 on the PCI side. If this is not true, we can end up with I/O port numbers in the resources for PCI devices which will cause an oops if a driver tries to access them via inb/outb etc., because there is no mapping for the corresponding addresses. Normally the I/O window starts at 0, but there are some situations on partitioned machines with a hypervisor where the window may not start at 0. This fixes the problem by allocating space for the range from 0 to the end of the I/O window. That is, hose->io_base_virt contains the virtual address for I/O port 0 on the PCI bus, and thus the assumption that hose->io_base_virt - pci_io_base is the offset between the "global" I/O port numbers (those in the PCI device resources) and the I/O port numbers on the PCI bus is maintained. For PCI host bridges that are present at boot, we only map the portion of that range that correspond to the bridge's I/O window. For bridges added after boot we ioremap the range from 0 to the end of the I/O window, for now; in fact hot-added bridges should be using reserve_phb_iospace() and __ioremap_explicit (so they get sensible global port numbers), but we don't have the infrastructure yet to do that (basically a free_phb_iospace() routine plus appropriate locking). Interestingly, this makes the two arms of the if statement in get_bus_io_range do almost exactly the same thing; that function could now be simplified in a further patch. [POWERPC] Fix mmap of PCI resource with hack for X The powerpc version of pci_resource_to_user() and associated hooks used by /proc/bus/pci and /sys/bus/pci mmap have been broken for some time on machines that don't have a 1:1 mapping of devices (basically on non-PowerMacs) and have PCI devices above 32 bits. This attempts to fix it as well as possible. The rule is supposed to be that pci_resource_to_user() always converts the resources back into a BAR values since that's what the /proc interface was supposed to deal with. However, for X to work on platforms where PCI MMIO is not mapped 1:1, it became a habit of platforms like powerpc to pass "fixed up" values there since X expects to be able to use values from /proc/bus/pci/devices as offsets to mmap of /dev/mem... So we keep that contraption here, causing also /sys/*/resource to expose fully absolute MMIO addresses instead of BAR values, which is ugly, but should still work as long as those are only used to calculate alignment within a page. X is still broken when built 32 bits on machines where PCI MMIO can be above 32-bit space unfortunately. [POWERPC] Allow hooking of PCI MMIO & PIO accessors on 64 bits This patch reworks the way iSeries hooks on PCI IO operations (both MMIO and PIO) and provides a generic way for other platforms to do so (we have need to do that for various other platforms). While reworking the IO ops, I ended up doing some spring cleaning in io.h and eeh.h which I might want to split into 2 or 3 patches (among others, eeh.h had a lot of useless stuff in it). A side effect is that EEH for PIO should work now (it used to pass IO ports down to the eeh address check functions which is bogus). Also, new are MMIO "repeat" ops, which other archs like ARM already had, and that we have too now: readsb, readsw, readsl, writesb, writesw, writesl. In the long run, I might also make EEH use the hooks instead of wrapping at the toplevel, which would make things even cleaner and relegate EEH completely in platforms/iseries, but we have to measure the performance impact there (though it's really only on MMIO reads) Since I also need to hook on ioremap, I shuffled the functions a bit there. I introduced ioremap_flags() to use by drivers who want to pass explicit flags to ioremap (and it can be hooked). The old __ioremap() is still there as a low level and cannot be hooked, thus drivers who use it should migrate unless they know they want the low level version. The patch "arch provides generic iomap missing accessors" (should be number 4 in this series) is a pre-requisite to provide full iomap API support with this patch. [POWERPC] Resolve the parent address of a PCI bus range When parsing the OF "ranges" properties of PCI host busses to determine the mapping of a PCI bus, we need to translate the "parent" address using the prom_parse.c routines in order to obtain a CPU physical address. This wasn't necessary while PCI busses were always at the root of the device-tree but this is no longer the case on Cell where they can be anywhere in the tree. [POWERPC] Refactor 64 bits DMA operations This patch completely refactors DMA operations for 64 bits powerpc. 32 bits is untouched for now. We use the new dev_archdata structure to add the dma operations pointer and associated data to struct device. While at it, we also add the OF node pointer and numa node. In the future, we might want to look into merging that with pci_dn as well. The old vio, pci-iommu and pci-direct DMA ops are gone. They are now replaced by a set of generic iommu and direct DMA ops (non PCI specific) that can be used by bus types. The toplevel implementation is now inline. [POWERPC] Make pci_read_irq_line the default This patch reworks the way IRQs are fixed up on PCI for arch powerpc. It makes pci_read_irq_line() called by default in the PCI code for devices that are probed, and add an optional per-device fixup in ppc_md for platforms that really need to correct what they obtain from pci_read_irq_line(). It also removes ppc_md.irq_bus_setup which was only used by pSeries and should not be needed anymore. I've also removed the pSeries s7a workaround as it can't work with the current interrupt code anyway. I'm trying to get one of these machines working so I can test a proper fix for that problem. I also haven't updated the old-style fixup code from 85xx_cds.c because it's actually buggy :) It assigns pci_dev->irq hard coded numbers which is no good with the new IRQ mapping code. It should at least use irq_create_mapping(NULL, hard_coded_number); and possibly also set_irq_type() to set them as level low. [PATCH] powerpc: fix trigger handling in the new irq code This patch slightly reworks the new irq code to fix a small design error. I removed the passing of the trigger to the map() calls entirely, it was not a good idea to have one call do two different things. It also fixes a couple of corner cases. Mapping a linux virtual irq to a physical irq now does only that. Setting the trigger is a different action which has a different call. The main changes are: - I no longer call host->ops->map() for an already mapped irq, I just return the virtual number that was already mapped. It was called before to give an opportunity to change the trigger, but that was causing issues as that could happen while the interrupt was in use by a device, and because of the trigger change, map would potentially muck around with things in a racy way. That was causing much burden on a given's controller implementation of map() to get it right. This is much simpler now. map() is only called on the initial mapping of an irq, meaning that you know that this irq is _not_ being used. You can initialize the hardware if you want (though you don't have to). - Controllers that can handle different type of triggers (level/edge/etc...) now implement the standard irq_chip->set_type() call as defined by the generic code. That means that you can use the standard set_irq_type() to configure an irq line manually if you wish or (though I don't like that interface), pass explicit trigger flags to request_irq() as defined by the generic kernel interfaces. Also, using those interfaces guarantees that your controller set_type callback is called with the descriptor lock held, thus providing locking against activity on the same interrupt (including mask/unmask/etc...) automatically. A result is that, for example, MPIC's own map() implementation calls irq_set_type(NONE) to configure the hardware to the default triggers. - To allow the above, the irq_map array entry for the new mapped interrupt is now set before map() callback is called for the controller. - The irq_create_of_mapping() (also used by irq_of_parse_and_map()) function for mapping interrupts from the device-tree now also call the separate set_irq_type(), and only does so if there is a change in the trigger type. - While I was at it, I changed pci_read_irq_line() (which is the helper I would expect most archs to use in their pcibios_fixup() to get the PCI interrupt routing from the device tree) to also handle a fallback when the DT mapping fails consisting of reading the PCI_INTERRUPT_PIN to know wether the device has an interrupt at all, and the the PCI_INTERRUPT_LINE to get an interrupt number from the device. That number is then mapped using the default controller, and the trigger is set to level low. That default behaviour works for several platforms that don't have a proper interrupt tree like Pegasos. If it doesn't work for your platform, then either provide a proper interrupt tree from the firmware so that fallback isn't needed, or don't call pci_read_irq_line() - Add back a bit that got dropped by my main rework patch for properly clearing pending IPIs on pSeries when using a kexec Signed-off-by: Benjamin Herrenschmidt <benh@kernel.crashing.org> Cc: Paul Mackerras <paulus@samba.org> Signed-off-by: Andrew Morton <akpm@osdl.org> Signed-off-by: Linus Torvalds <torvalds@osdl.org>

[POWERPC] Add new interrupt mapping core and change platforms to use it This adds the new irq remapper core and removes the old one. Because there are some fundamental conflicts with the old code, like the value of NO_IRQ which I'm now setting to 0 (as per discussions with Linus), etc..., this commit also changes the relevant platform and driver code over to use the new remapper (so as not to cause difficulties later in bisecting). This patch removes the old pre-parsing of the open firmware interrupt tree along with all the bogus assumptions it made to try to renumber interrupts according to the platform. This is all to be handled by the new code now. For the pSeries XICS interrupt controller, a single remapper host is created for the whole machine regardless of how many interrupt presentation and source controllers are found, and it's set to match any device node that isn't a 8259. That works fine on pSeries and avoids having to deal with some of the complexities of split source controllers vs. presentation controllers in the pSeries device trees. The powerpc i8259 PIC driver now always requests the legacy interrupt range. It also has the feature of being able to match any device node (including NULL) if passed no device node as an input. That will help porting over platforms with broken device-trees like Pegasos who don't have a proper interrupt tree. [PATCH] mm: remove VM_LOCKED before remap_pfn_range and drop VM_SHM Remove VM_LOCKED before remap_pfn range from device drivers and get rid of VM_SHM. remap_pfn_range() already sets VM_IO. There is no need to set VM_SHM since it does nothing. VM_LOCKED is of no use since the remap_pfn_range does not place pages on the LRU. The pages are therefore never subject to swap anyways.
