powerpc/ps3: remove orphan declarations from ps3av.h

Remove the following orphan declarations from ps3av.h:
1. ps3av_dev_open()
2. ps3av_dev_close()

They have been removed since commit 13a5e30cf740 ("[POWERPC] PS3:
Rework AV settings driver"), so remove them.

Change the ps3av_auto_videomode() mode id argument type from unsigned to
signed so a negative id can be detected and reported as an -EINVAL failure.

powerpc/ps3: Replace the flip_ctl logic in ps3av and ps3fb by a mutex

Introduce ps3_gpu_mutex to synchronizes GPU-related operations, like:
- invoking the L1GPU_CONTEXT_ATTRIBUTE_FB_BLIT command using the
lv1_gpu_context_attribute() hypervisor call,
- handling the PS3AV_CID_AVB_PARAM packet in the PS3 A/V Settings driver.

ps3: Add passthru support for non-audio streams

Add support for the channel status bit setting so that non-PCM
data stream can be sent (i.e. pass-through) via SPDIF/HDMI.

powerpc: Move include files to arch/powerpc/include/asm

from include/asm-powerpc. This is the result of a

mkdir arch/powerpc/include/asm
git mv include/asm-powerpc/* arch/powerpc/include/asm

Followed by a few documentation/comment fixups and a couple of places
where <asm-powepc/...> was being used explicitly. Of the latter only
one was outside the arch code and it is a driver only built for powerpc.
