add-ons/kernel: add spaces between literals and identifiers

Due to introduction of user-defined suffixes C++11 requires that there
is a space between literal and identifier to avoid ambiguity.

This patch makes the whole kernel build successfully with C++11.

Remove B_COMMON*_DIRECTORY and kCommon*Directory constants

* This does intentionally break source compatibility, so that a review
of concerned code is forced.
* Binary compatibility should be maintained in most cases. The values
of the constants for the writable directories are now used for the
writable system directories. The values for the non-writable
directories are mapped to "/boot/system/data/empty/...", an empty or
non-existent directory, so that they will simply be skipped in search
paths. Only code that explicitly expects to find something in a
B_COMMON_* directory, will fail.

Filtered flat import of Oliver's svn package management branch

Bring the changes that aren't package management related and the ones
that are but don't take effect as long as they are ignored by the build
system into the master.

Summary of changes:
* Introduce private header <directories.h> with constants for a good
deal of paths that should usually be retrieved via find_directory().
* Replace hard-coded paths by using find_directory() or the
<directories.h> constants (e.g. in drivers and the kernel).
* Add find_directory() constants needed for package management.
* Add __HAIKU_ABI_NAME and B_HAIKU_ABI_NAME macros.
* src/apps/deskbar: BeMenu.* -> DeskbarMenu.*,
DeskBarUtils.* -> DeskbarUtils.*
* Change deskbar menu settings directory from ~/config/be to
~/config/settings/deskbar.
* Other smaller cleanups, changes, and fixes.

* Print the speeds even though they are marked as not used (as the baud index
is inside the cflags) since stty for example sets them.
* Turn off debug output by default and fix a warning due to that.

Fix gcc4 build:
- use new(std::nothrow) from <new> since we dropped kernel_cpp.h,
- fix "declared 'extern' and later 'static'" warning,
- fix "deprecated conversion from string constant to 'char*'" warning in tracing code.

Port of usb_serial to the generic tty module. I don't remember exactly if or how
well this was working, but since it was non-functional on Haiku anyway this
shouldn't hurt.

- change KLSI init the way it's done by the linux driver
- do not count usb headers as part of count returned by write(), else we might end up writing more than the passed amount :)

Rework of the usb_serial driver:
* Refactored everything to C++ with the different devices as subclasses
* Added proper ACM detection with parsing of the ACM descriptors
* Added device transfer error handling and fixed some concurency issues
* Big cleanup to conform to our style guide

This should make at least ACM stable to use. Commiting this over my K850i with
ACM compliant USB modem and UMTS data connection. Note that support for all
other device classes (Prolific, FTDI and KLSI) is untested but should work the
same as before. Note also that since we currently lack a TTY module this will
only build/work for R5 or Dano with the proper TTY headers.
