radeon_hd: Fixes for SMAP.

Only partially "tested", but I've audited this code pretty thoroughly, and
it seems the accelerant uses clone_area instead of direct pointer accesses
in all the correct places, so this should work.

* Don't raise the open_count when the open fails
* Remove the kdl command on uninit

* open_count only got raised on initial open,
subsequent opens didn't increment the counter
thus when they exited the driver unloaded
causing app_server to crash

the if checks the pre-increment number
* Resolves #10452
* Add a kdl debug command to radeon_hd

Work in progress of a Radeon HD 3200 driver to change the display mode (status when I leave the coding spring). At least it shows a picture but with a wrong scaling.
