mdoc: order prologue macros consistently by Dd/Dt/Os

Remove single-space hard sentence breaks. These degrade the quality
of the typeset output, tend to make diffs harder to read and provide
bad examples for new-comers to mdoc.

Make buf in authflags a little bigger because 2 strings can be
concatenated and 20 bytes (the old size) is kind of tight.
